If you cannot find support in the community, we can carry out a needs assessment to see if we can support you.
At this meeting you can explain to us what care and support you feel you need.
If your difficulties meet the criteria, we can offer support.
Questions may include:
- What care tasks can you do for yourself?
- What care tasks do you need help with?
- What care tasks are you unable to carry out?
- What help do you have currently?
- What is the best way to provide the support you need?
You are welcome to invite someone along to support you.

After the assessment
After the assessment, we will decide whether you are eligible for support.
If support is agreed we will create a care and support plan.
The care and support plan will give details of:
- services you will receive
- when and how often you will receive them
- names of key staff members
- date when the services will be reviewed
- needs which cannot be met and the reasons why
- what to do if you are unhappy with the service
We will give you a copy of the plan and discuss it with you.
If you do not qualify for care and support, we can still give you information and advice about where you can get help in your community.
