To be eligible for support from us, you need to meet all three of these conditions:
- Your needs are related to a physical or mental impairment or illness.
- As a result, you are unable to achieve two or more of the following:
- Maintain nutrition – access food or drink to remain in good health
- Maintain personal hygiene – wash yourself and your own clothes
- Manage toilet needs
- Clothe and dress yourself – such as dressing suitably for the weather
- Use your home safely – use the kitchen and stairs, and enter and leave your property, safely
- Maintain a clean and safe home and access essentials such as water, electricity, and gas
- Develop and maintain family or other personal relationships – and develop new ones
- Access work, training, education, or volunteering
- Make use of community services, such as public transport and access recreational spaces (this does not include NHS patient transport but may include your ability to attend healthcare appointments)
- Carry out caring responsibilities for a child – such as parenting
- As a result of being unable to achieve these there is, or there is likely to be, a significant impact on your wellbeing.
