We offer a range of services to people who are 18 years old or older with physical disabilities.
We aim to help you live in your own home and remain as independent as possible.
Services we offer
- Information and advice, including Merton Community Hub. (You can access Merton Community Hub services without needing to have a care assessment from us.)
- Equipment and adaptations to your home to make you more independent
- Activities at one of our day centres
- Short-term help at home (reablement) following hospital discharge and help leaving hospital
- Support to access employment
- Care in your own home if you are eligible
- Occupational therapy services to enable you live as independently and as safely as possible in your own home
- Direct payments to support independent living
- Carer’s parking permits
Eligibility
For some of these services you will need to have a needs assessment. Our team will assess your needs to see what support we can offer you.

For monitoring support in your home you can sign up for Mascot Telecare community alarm service. If you need help getting your voice heard there is independent advocacy (someone to speak up for you), and you can search our Adult Support Services Directory for lots of other support services.
