Creating an emergency plan
It is important to create an emergency plan in case you are not able to support the person you care for.
The plan should include:
- name, address, and any other contact details of the person you care for
- emergency contact details of people who can provide replacement care
- any medication the person is taking
- any ongoing treatment they need
- their support, mobility and communication needs
You should:
- keep it in a safe place where it can be found in an emergency
- share copies with your emergency contacts, GP, and the social worker of the person you care for, if they have one
- keep it updated throughout your life.

The person you care for can also create a Universal Care Plan which will ensure their care and support wishes are digitally shared with healthcare professionals across the capital.
The plan can be reassuring and useful in an emergency when a carer may not be present.
