Introduction
We have different forms for claiming Housing Benefit (HB) Council Tax Support (CTS), Discretionary Housing Payment (DHP) and for reporting changes in circumstances. We also have forms if you need to give us proof of your income with your claim or report a change or address in Merton. These can be found below where you can download and print for use.
Claim forms (new claims and new address)
Housing Benefit and Council Tax Support
If you want to make a new claim for HB or CTS, you should firstly check New Claims for Housing Benefit and Claiming Council Tax Support and check if you need to fill in a form.
If you do need to fill in a claim for HB please use our HB and CTS on line claim form. The on line form will guide you through the application. You can access this for free 24 hours a day 7 days a week. You can access this from Merton Libraries during their usual opening hours.
If you need a paper form you can download and print our Housing Benefit and Council Tax Support form but please note that unlike the on line claim form where your claim will start on the date you start filling it in, a paper claim will start from the Monday after we receive the form. You may lose benefit using this method.
Discretionary housing payments
The form you need to fill in for DHP depends on whether you are claiming Housing Benefit or Universal Credit to get help with your rent. For more information and the forms to download and print, see Discretionary housing payments.
Reporting changes
Reporting changes in income or savings for you and household members

Landlords reporting tenants in arrears
If a tenant is more than eight weeks in arrears we must pay the landlord directly. Please use our online form to report your tenant is more than eight weeks in arrears. See our Paying benefits page for more information about this.
Landlords requesting direct payments for vulnerable tenants
Under Local Housing Allowance rules, we can consider paying landlords or their agents, where the tenant is likely to have difficulty paying their rent. Please use our online request form to request this. See our Paying benefits page for more information about this.
